Title V inspection when purchasing real estate from a family member

When purchasing real estate (a house) from a direct family member (brother), does it need to pass a home inspection and TITLE V. We have been living in the house for years and paying the mortgage, but everything is still in my brother's name.

A home inspection is something done voluntarily by the buyer to make sure the home is sound and that there are no hidden problems that could lead to huge expenses.  So, it is up to you whether you have such an inspection done.
 
A Title V inspection, however, is required whenever a home is sold.  I do not know of any exceptions for sales of real estate between family members, but the MA DEP has an excellent website with FAQs and other helpful information:  http://www.mass.gov/dep/water/wastewater/faqsgen.htm
