Last will and testament from 1991

Our decedent's will dates from 1991; in it, she assigns two co-executors. We are applying for an informal probate of will and have a few questions:

Can one of the co-executors simply renounce the position on the "Assent and Waiver of Notice/Renunciation/Nomination Waiver" form (#MPC455) as a routine part of the filing procedure?

Is the executor automatically appointed personal representative by a simple change of nomenclature when the will is filed with the court, or is some sort of petition required to be appointed personal representative?
