Where can I get a standard form to send a 14-day notice to quit for non-payment (for a tenant at will)?

I am looking for a standard form to send a 14 day notice to quit for non-payment. My rental is a tenant at will, based upon the original term having expired and we are now in a month to month arrangement. I went to Staples and they do not have anything. The Housing Court wesite indicates there is a difference in the document to used for a tenant at will as compared to a tenant under lease. I found one online but they do not differentiate between the two rental agreement status'. Thoughts on where to find this document? (Electronically preferred.)
