Is my rental agreement a lease or a month-to-month?

I'm a little confused about the language of my lease... I should note that I signed it and have lived in this apartment for a year and a half, but am considering moving. I don't want to break my lease, if it is indeed a year-long lease. Here's the relevant portion (typos are left as they appear in the document):

4. TERM OF LEASE AGREEMENT:
The Lease will begin on JUNE 01, 2009, and continue on a month to month basis by JUNE 01, 2010 and proper notice is given.

22. ENDING OR REVEWING THE LEASE AGREEMENT:
Either the Landlord or the Tenant may end or change the terms of this lease by giving 60 days prior written notice.

When my landlady referred to my lease verbally, she called it a one-year lease, and she asked me to sign a new agreement (identical to the first) last June after my first year of tenancy was up. The wording of the above, however, makes me think that it's a month-to-month, or some strange combination of the two. Does anyone have any idea which category this falls into, and what my rights might be with regards to moving out before June 2010?

That is strange.  I'll assume this is document the landlord created, rather than a standard form.  I certainly cannot tell without looking at the whole document, but that language about ending the 'lease' with 60 days notice sure sounds like it belongs in a tenancy-at-will agreement.  Leases, by their very nature, cannot be cancelled with notice.  They continue for a set term.
