How do I get a handicap parking plate or placard?

I have a really hard time breathing and often can't walk more than a few feet at a time. How do I qualify for handicap parking plate or placard in Massachusetts? And if I'm qualified how do I get one? Is this something my doctor can do for me? Thanks.

To qualify, you must get a licensed doctor, nurse practitioner or chiropractor to certify that you have certain medical conditions such as you: (1) are unable to walk 200 feet without stopping to rest; (2) are unable to walk without the help another person or prosthetic aid; (3) use portable oxygen; (4) have certain cardiac conditions; (5) have certain forms of arthritis; (5) are legally blind (must surrender your license); or (6) have lost one or more limbs or permanently or lost the use of one or more limbs.  Your health care professional will help you to determine if you are qualified because of these or some other condition.  But based on what you say, it seems as though you will qualify. 
 
For a temporary placard the health professional must certify that your condition is likely to last at least two months.
 
To get an application, call the Registry of Motor Vehicles at 1-(800) 858-3926.  After you and your health professional have filled out the application, take it to the nearest full service RMV office or mail it to:
