Court Ordered Child Support and Voluntary Relinquishment of Paternal Rights

My daughter lives with my mom in Mass, I'm in the military living in Florida and because of my ships schedule and duty days I can't care for my daughter as a single mom. I send my mom money every month for my daughters expenses. Her biological dad is supposed to be sending a certain amount by mutual agreement, and I just now found out he's not sending the amount we agreed upon. Also, he emailed my mom basically saying flat out he didn't want to be my daughters father anymore. That's all fine for me, doesn't bother me. My question is, 1. How do I go about getting a court order to take him off the birth certificate (my daughter was born in Georgia) and relinquish all rights as a parent? 2. Can I get my daughters name changed at the same time? and 3. Because I'm in the military, do I file in the state I'm currently living in, or the state my daughter resides in?
